A pain in the neck: probiotics for ulcerative colitis.

In this patient suffering from inflammatory bowel diseases bacteria originating from probiotics were found in epidural abscesses. The resistance pattern of lactobacillus rhamnosus indicated the origin of probiotics. This case reports illustrates that the use of probiotics in medical practice deserves close attention and careful individual evaluation, in particular in patients with immune suppression.

Impact of a national HIV voluntary counselling and testing (VCT) campaign on VCT in a rural hospital in Tanzania

In this study Mossdorf and collaborators investigate the impact of a national HIV voluntary counselling and testing campaign on the overall voluntary counselling and testing in a rural area in Tanzania. The authors found that the campaign had a beneficial effect on the frequency of testing. Interestingly, the rate of positive tests was much higher than the overall prevalence countrywide. This illustrates that with campaigns individuals with higher probability to test positive are motivated to participate in VCT.

In a state of the art-article Widmer et al summarize surgical hand preparation and how the use of alcohol-based hand rubs for surgical hand preparation is clearly preferable.

Not all patients with vancomycin-resistant enterococci need to be isolate

Tschudin Sutter et al. describe with strong evidence that carriers of Vancomycin-resistant enterococci (VRE) of genotype vanC do not require contact isolation, thereby saving resources and potentially improving patient care. The study concludes that the genotype should be routinely determined in areas with a high prevalence of VRE of genotype vanC.

Virosomal influenza-vaccine induced immunity in HIV-infected individuals with high versus low CD4+ T-cell counts

This study led by C. Hess provides evidence that establishing broad influenza-specific B-cell memory prior to severe immunodeficiency is important.

A rifampicin-containing antibiotic treatment improves outcome of staphylococcal deep sternal wound infection

This study demonstrates that treatment with rifampicin-susceptible staphylococcal deep sternal wound infections with a rifampicin-containing antibiotic regimen may improve the outcome. After surgical debridement an antibiotic treatment of 6 weeks may be adequate for staphylococcal deep sternal wound infections.

Treatment modification in HIV-infected individuals starting combination antiretroviral therapy between 2005 and 2008

This Swiss HIV Cohort Study by Elzi et al. demonstrates that drug toxicity remains a frequent reason for treatment modification. However, it does not affect treatment success. Importantly, close monitoring and management of adverse effects and drug-drug interactions are crucial for the durability of antiretroviral treatment.
